All submissions must be completed online through this form. A committee will be reviewing the submissions and selecting the honorees. Honorees will be notified by July 31, 2024. We request that honoree information be kept private and internal within your organization until after the event concludes. We request all honorees be available for the evening of OCBC’s First Responders Dinner at the Westin Anaheim Resort on September 12, 2024 between 6-8pm. During the event, the honoree’s story will be read aloud, and then the honoree will be awarded a plaque on stage and take a quick photograph with their CEO or immediate supervisor on stage.

Selection Committee Criteria:

The selection committee fully understands that the calling for first responders is to save lives. In some circumstances, that call can go above and beyond. The selection committee is looking for entries that:

A. Demonstrate first responders going above and beyond their call of duty in extreme and gripping circumstances.

B. Demonstrate powerful stories that convey the risk first responders take and their vital importance to keeping our community healthy and safe.

Please note, if the nominee is selected as an honoree, the following information entered in this section will be exactly how it will be printed in the event program, appear on the screens at the event, on the event plaque, on social media posts and OCBC’s website, and anywhere else the honorees may be listed on their employer’s communications channels.
